You could see your doctor and get a prescription for Ketoconazole or zinc pyrithione shampoo.

For severe infections, Fluconazole (Diflucan), Itraconazole and especially Voriconazole might work. These are usually taken by mouth so continuing external treatment is best.

I have had good luck with liquid grapefruit seed extract blended with olive oil so it doesn't burn your skin as well.
